Is not seeking recreational, medical, or consciousness-altering experiences for secular purposes
Acknowledges the legal gravity of falsely seeking access to controlled substances through deceit, including under 21 U.S.C. § 843(a)(3)

Contract for Entheogenic Facilitation Services (Contract Form A)
(Facilitator ↔ Church/Organization Form – Institutional Use – $555.00)
This contract form is tailored for use between a solo entheogenic facilitator and a sponsoring religious organization, nonprofit, or church. It governs the provision of entheogenic facilitation services in the context of religious ceremonies and rituals, and is intended to support lawful, safe, and sincere religious use under RFRA and applicable state analogues.
Key provisions include:
Introduction of the Parties – Identifies both the facilitator and the organizational client (e.g., church or nonprofit), including legal structure and authorized representative.
Statement of Shared Religious Intent – Affirms the mutual religious nature of the agreement and links the ceremony to each party’s sincerely held beliefs and practices.
Diversion & Harm Prevention Duties – Acknowledges shared responsibilities to minimize risk of sacrament diversion and participant harm, and outlines behavior standards for both parties.

1. Copyright Notice & Liquidated Damages
All course materials, including but not limited to the EntheoEsq Facilitator Forms Suite and related documents, are the exclusive property of EntheoEsq, LLC and are protected under U.S. copyright law (17 U.S.C. § 101 et seq.) and international treaties. Any unauthorized reproduction, distribution, public display, resale, or creation of derivative works, in whole or in part, is strictly prohibited and will be enforced to the fullest extent of the law.
By purchasing these documents you acknowledge that these materials are licensed solely for your personal and direct professional use, and that any unauthorized use or disclosure would cause substantial harm to EntheoEsq, LLC, including but not limited to lost licensing revenue, diminished exclusivity, and reputational damage.
You further agree that, because the precise amount of such harm would be difficult to calculate, liquidated damages in the amount of $150,000.00 per occurrence, with a maximum aggregate of $500,000.00, constitute a fair, reasonable, and agreed-upon pre-estimate of damages, and are not a penalty.
