This critical form is signed by the prospective congregant or participant and serves as a formal declaration of sincere religious intent prior to any entheogenic ceremony. It is designed to filter out recreational users, thrill-seekers, and bad actors, ensuring that sacraments are only dispensed within a bona fide religious framework.

The Certification of Intent form certifies that the participant:

  • Understands the religious and spiritual nature of the ceremony

  • Has appropriate, lawful intentions aligned with RFRA and First Amendment protections

  • Is not seeking recreational, medical, or consciousness-altering experiences for secular purposes

  • Acknowledges the legal gravity of falsely seeking access to controlled substances through deceit, including under 21 U.S.C. § 843(a)(3)

Facilitators use this form as a front-line defense against misuse and a foundational element of their religious liberty compliance framework.

    1. Copyright Notice & Liquidated Damages

    All course materials, including but not limited to the EntheoEsq Facilitator Forms Suite and related documents, are the exclusive property of EntheoEsq, LLC and are protected under U.S. copyright law (17 U.S.C. § 101 et seq.) and international treaties.     By purchasing these documents you acknowledge that these materials are licensed solely for your personal and direct professional use, and that any unauthorized use or disclosure would cause substantial harm to EntheoEsq, LLC, including but not limited to lost licensing revenue, diminished exclusivity, and reputational damage.

    You further agree that, because the precise amount of such harm would be difficult to calculate, liquidated damages in the amount of $150,000.00 per occurrence, with a maximum aggregate of $500,000.00, constitute a fair, reasonable, and agreed-upon pre-estimate of damages, and are not a penalty.
